The giveaway is open to legal residents of the United States who are 18 years of age or older at the time of entry. Employees of Company 119, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members are not eligible to participate.
To enter, participants must fully complete and submit the giveaway entry form provided by Company 119 at the ACCA & PHCC Annual Golf Outing or through the designated online entry form. Limit one (1) entry per person. Duplicate or incomplete entries will be disqualified.
One (1) winner will be selected to receive a premium golf bag. No substitution of prize or transfer/assignment of prize to others is permitted. No cash equivalent will be offered.
The winner will be selected in a random drawing conducted by Company 119 following the close of the event. The winner will be notified via the contact information provided in the entry form. If the winner does not respond within seven (7) days of notification, an alternate winner may be selected.
By entering, participants agree to be bound by these Official Rules and the decisions of Company 119, which are final and binding. The winner may be required to sign and return an affidavit of eligibility and a liability/publicity release, as permitted by law.
Personal information collected through the entry form will be used solely for administering the giveaway and will be handled in accordance with Company 119’s Privacy Policy.
